Examples: parent and child, spouses, relatives, or friends.

Vehicle Information

Enter the full VIN exactly as shown on the title.
Enter the model year.
Example: Toyota, Ford, or Honda.
Enter the model name.
Enter the current mileage without commas.
Enter the title number when available.
Optional plate information.

Gift and Transfer Details

Enter the intended transfer date.
State that the vehicle is being transferred as a genuine gift and describe any relevant facts.
Enter “None” if no payment or other value was exchanged.
List relevant details, but confirm requirements with the motor vehicle agency.
Add only facts the parties know to be true.
