What to have ready
- The mailbox provider’s address and PMB number.
- The applicant’s full name, contact details, and permanent home address.
- One valid government-issued photo ID.
- A second current document that verifies the home address.
- Business information or an authorized individual’s information when applicable.
Important completion notes
- Complete all entries requested by the mailbox provider.
- The applicant must sign or confirm the signature in real-time presence of the CMRA or a qualified notary.
- Copies of the required photo and address identification are generally attached.
- The June 2024 form requires separate adult applications for residential or personal use.
