The document or receiving organization may request additional evidence, an address, a tax identifier, immigration information, or another transaction-specific item that is separate from notarial identification.

Compare the name on the document with the name on the ID. Common issues include omitted middle names, different surname order, married or former names, hyphenation, suffixes, initials, transliteration from another alphabet, and minor spelling differences.
The notary cannot casually rewrite the document or decide which legal name should be used. Contact the document preparer or receiving organization when the name format is unclear. If a name change has occurred, ask whether supporting documentation or a corrected form is required.
Accents, apostrophes, hyphens, and multiple surnames should be entered accurately in the booking intake and document. Do not remove meaningful characters merely to simplify the form.
