A completed authorization commonly includes the child’s full name, date of birth or student ID when requested, the organization’s name, the authorized adult’s exact legal name, relationship to the child, phone number, effective date, expiration date, and any one-time or recurring schedule. The organization may also request a photograph, copy of identification, password, vehicle information, or emergency contact.
Use the same spelling for the authorized adult that appears on the ID the person will show at pickup. If multiple adults are approved, list each one clearly. Do not use vague descriptions such as “family member” when the organization requires specific names.
